Biliary stent placement, while generally safe and effective, carries certain risks and potential complications. These can occur during the procedure or afterward and may vary depending on the patient’s overall health, the underlying condition being treated, and the type of stent used. Here’s an overview of the key risks and complications:


### 1. **Infection:**

- **Cholangitis:** This is an infection of the bile ducts that can occur if bacteria enter the bile ducts during the procedure. Symptoms include fever, chills, abdominal pain, and jaundice.

- **Bacteremia:** In rare cases, bacteria may enter the bloodstream, leading to a serious infection called sepsis.


### 2. **Pancreatitis:**

- Inflammation of the pancreas, or pancreatitis, can occur after ERCP. This is one of the more common complications, with symptoms such as severe abdominal pain, nausea, vomiting, and fever.


### 3. **Bleeding:**

- The procedure can cause bleeding, especially if the bile duct or surrounding tissues are damaged during stent placement. This is more likely if the patient has a blood clotting disorder or if a biopsy is performed during the ERCP.


### 4. **Perforation:**

- There is a small risk of perforation (a tear or hole) in the bile ducts, intestines, or stomach during the procedure. This can lead to leakage of bile or intestinal contents into the abdominal cavity, which may require surgical repair.


### 5. **Stent-Related Complications:**

- **Stent Migration:** The stent can move from its original position, potentially causing bile duct blockage, intestinal obstruction, or other complications.

- **Stent Blockage:** Over time, the stent may become blocked by bile sludge, debris, or tumor growth, leading to recurrent symptoms such as jaundice or infection.

- **Stent Fracture:** Rarely, metal stents can break, which may necessitate additional procedures for removal or replacement.


### 6. **Allergic Reaction:**

- Some patients may have an allergic reaction to the contrast dye used during ERCP, anesthesia, or materials in the stent. Symptoms can range from mild (rash, itching) to severe (anaphylaxis).


### 7. **Bile Leak:**

- A bile leak can occur if the bile duct is damaged during stent placement or if the stent does not fully cover a tear or hole. This can cause pain, infection, and inflammation in the abdominal cavity (peritonitis).


### 8. **Recurrence of Symptoms:**

- Even with successful stent placement, symptoms such as jaundice, itching, and abdominal pain may recur if the stent becomes blocked or if the underlying condition (e.g., tumor growth) progresses.


### 9. **Sedation or Anesthesia Complications:**

- Some patients may experience adverse reactions to sedation or anesthesia, such as breathing difficulties, low blood pressure, or allergic reactions.


### 10. **Death:**

- While extremely rare, there is a small risk of death associated with biliary stent placement, particularly in very ill patients or those with complex medical conditions.

- **Pre-Procedure Assessment:** Careful evaluation of the patient's medical history, medications, and overall health can help reduce risks.

- **Antibiotics:** Prophylactic antibiotics may be given to prevent infection.

- **Monitoring:** Close monitoring during and after the procedure helps in early detection and management of any complications.


While biliary stent placement can have risks, it is often a necessary and life-saving procedure for managing bile duct obstructions, with most patients experiencing significant relief and improved quality of life.
