There are a few different techniques that can be used to remove tracheal stents:
- Rigid bronchoscopy - This is the most common approach. The patient is under general anesthesia, and a rigid tube is inserted into the trachea to visually access the stent. Tools can be passed through the bronchoscope to grasp and extract the stent.
- Cryoextraction - A cryoprobe is used to freeze the stent, adhering it to the probe so it can be pulled out. Helpful for stents that are stuck or embedded.
- Laser resection - Laser fibers are inserted through the bronchoscope to carefully cut away and collapse parts of the stent for easier removal. This may be needed for metal or very rigid stents.
- Stent cutters - Specialty forceps with cutting blades can bite into and cut up stents to facilitate removal in pieces if needed.
- Balloon dilation - Balloons are sometimes used to re-expand a collapsed stent for easier grasping.
- Open surgery - Rarely needed, but tracheal resection to remove very adherent stents may be a last resort if endoscopic techniques fail.
The specific technique depends on the stent type and how much tissue growth has occurred. Silicone stents often can be directly grasped and pulled out. Heavily ingrown metal or hybrid stents usually require additional techniques like lasers. Skilled operators are crucial for safe stent removal.
